Form F37 Child Support Affidavit is a prescribed form under the Supreme Court Family Rules. It is used in both joint and sole claims for divorce that are uncontested. Because the same form is completed differently depending on whether a joint or sole claim is involved, be careful to ensure you use the correct example.
