Form F102 Statement of Information for Corollary Relief Proceedings is a form necessary to comply with Rule 15-2.2 of the Supreme Court Family Rules, which pertains to Orders for Corollary Relief in Divorce Proceedings. If either party is seeking a child support order, a spousal support order, or a parenting order under the Divorce Act, each party must complete this form and file a new one if the information changes. It is included with application records (Rule 10-6 (14)), applications for judgement in undefended applications under Rule 10-10 (see sub-rule (2)), trial records (Rule 14-4), and petition records (Rule 17-1).
